INSTRUCTIONS FOR NEW PATIENTS BEFORE THEIR 1ST APPOINTMENT
A. Fill out our forms online and email it to the respective clinic.
OR
B. Print out our forms at home and bring it to your visit.
1. Please bring your medical card, list of current medications. If the clinic is busy, you may have to wait.
2. If you are moving from a former clinic, please fill out the online Records Transfer Request Form and send it to your previous clinic/doctor. (You can print the PDF here).
3. Book a appointment by calling our clinic or booking online through here or through our online chat.
